On this entries form, you specify the details of the products that you are submitting to the Santé Wine & Spirits Competition. You must fill out this form, or we will not be able to process your entry. In addition, if you haven’t done so already, you need to pay for the number of entries on the payment page.
All entries must be paid for, recorded, and received at our storage facilities by July 15th. 2022.

Shipping Quantities and Label
Please provide the following quantities to ensure prompt and accurate judging:
- Wine: four 750 ml bottles (or equivalent if in smaller sizes).
- Spirits: two 750ml bottles (or equivalent).
- RTDs and cans: 8 of each.
Include with your products your name and email address as entered below so that we can match your entry with the actual product.
All entries are held in insured and climate-controlled facilities.
